Electrocardiogram of a 74-year old man with hypertension, CAD, previous MI, type 2 DM and
chronic AF.

Rate ~ 80/min
QRS 0.10 sec
QT 0.39 sec
QTc 0.46 sec
QRS axis +70º

Interpretation: AF with controlled ventricular rate. ST-T changes, "scooped" ST segment depression
in multiple leads suggestive of digitalis effect.

Electrocardiographic (electrophysiologic) effects of digitalis include prolongation of the PR interval, alterations of the ST segment and T waves. The T wave amplitude is lowered, ST segment depressed and shortened, QT interval shortened. Sagging or "scooped" ST segment depression can however be also produced by other causes.
