Electrocardiogram of an asymptomatic 23 year-old man, who had the test done prior to knee arthroscopy.

QRS axis +75º

Interptetation: Precordial leads recorded at half calibration. Abnormal. Left ventricular hypertrophy with repolarization abnormalities.

Although voltage criteria for LVH apply above the age of 40, large QRS voltage in young individuals is strongly suggestive of hypertrophy. Echocardiogram (panel below) showed hypertrophic cardiomyopathy with marked septal hypertrophy (2.6 cm), but no resting gradient in the left ventricular outflow tract.
